Scheduling an Appointment

• Step 1: Please complete and submit either the Canine/Feline or Equine New Client Questionnaire appropriate for your animal.

• Step 2: Please review, print, and sign the Treatment Authorization Form prior to your appointment.

• Step 3: Please contact your primary care veterinarian and/or specialist to request them to email a copy of you pet’s medical records to us (relevant medical history, vaccinations, & diagnostic tests are most helpful). Medical records can be emailed to info@thepioneervet.com.

We will contact you shortly after to schedule an appointment.

Please allow a 30 minute estimated time of arrival window

For equine appointments, please bathe your horse prior to the appointment

Service Range is 20 miles from Port Saint Lucie – additional travel fee may apply for further locations
